This will take care of all the intake and insurance information needed prior to the evaluation. After the evaluation, your child will receive a plan tailored to his/her needs, and will be assigned weekly therapy appointments, if needed. Please fill the following information to the best of your ability, and an administrator will reach out to you to set an initial evaluation within 24-48 hours.
